Home health services
You may need home health services after you leave the hospital. Home health care is generally provided on a short-term basis to monitor a patient’s medical condition and provide education in such areas as wound care management, home exercise programs, and intravenous (IV) medication administration.
You should consult with your primary care doctor if you need home health services. These services may not be a part of your insurance coverage, and you may need to pay for them yourself.
If you have questions about your benefits, please consult your case manager.
