Members of your health care team will help you plan your discharge. Your family or a person you designate also will be asked to participate in your discharge planning.

Your health care team will work with you to design a plan for your continued care outside the hospital.

The plan will include specifics such as:

• Use of medications and their potential side effects

• Diet instructions

• Activity level recommendations, exercise, or other activities

• Follow up appointments

Please ask questions and share concerns about your home situation with our staff before the day you are discharged.
